Overview

HACCPAC Care Plan

The HACCPAC Care plan is an integrated module for creating and managing client care plans. The Care Plan delivers a seamless view of client services across home care, meals and wellness management and planning. HACCPAC Care Plan has been created with feature requests from existing clients.

The Initial release focuses on major functional components, but it has been developed within a framework for ongoing feature enhancements.

Features

The first version of the Care Plan includes these great new features:

  • Care coordination forms checklist
  • Waiting list creation and management
  • Create multiple care plans per client
  • View all goals within an individual care plan
  • View the weekly care plan roster by household or client
  • View current and historical care plans
  • Schedule a task as weekly, fortnightly, monthly or other
  • View the weekly care plan roster by household or client
  • Allocate a Case Manager to a care plan
  • Create rostered meeting/client bookings for Case Managers
  • Create an appointments list per Case Manager by day and week
  • Specialised care plan reports using 'Reports on Demand'.

The new HACCPAC Care Plan module provides advanced care planning functionality using HACCPAC.

HACCPAC Care Plan is fully integrated with Haccpac, enabling powerful management reporting using the 'Reports on Demand' module.

Care Plan Checklist

The HACCPAC Care Plan includes the ability to add a user-defined Care Plan Checklist. For example, when a Care Plan is undertaken, forms a, b and c must be completed in order to finalise the Care Plan. Users can then tick the boxes confirming these forms have been completed.

Administrators can define care plan checklist items within the Utilities module. 

View Weekly Care Plan Roster

The HACCPAC Care Plan enables users to view the weekly Care Plan roster by household and client in a weekly calendar view. The Calendar displays rostered tasks, non-rostered tasks and case manager appointments.

Print Weekly Care Plan Roster

The HACCPAC Care Plan includes the ability to print a weekly Care Plan roster, including tasks allocated to the client, to provide to client and or third party. The printed weekly Care Plan roster includes Day of week, time and task information.

The weekly Care Plan roster can also be synchronised with Outlook.

Care Plan Goals

The HACCPAC Care Plan includes a table enabling an unlimited number of Care Plan goals to be added.  Goals can also be edited or deleted.

The Goals table includes Start, Review and End Date columns.

Goals can be viewed by type, i.e.:

  • Current goals
  • All goals.

The HACCPAC 'Reports on Demand' module includes the ability to report on goal review dates by client and by system wide care plans. 

Multiple Care Plans per Client

Using the HACCPAC Care Plan module, users can save multiple care plans per client. 

Existing Care Plans can be selected via a dropdown, or alternatively a new plan can be created by clicking the 'New' button.

Care Plan includes Start, Review and End Date

The HACCPAC Care Plan includes fields for Start, Review and End Dates.

Current and Historical Care Plans

The HACCPAC Care Plan allows users to select current or historical plans for a client via a dropdown list.

Review dates and Update Dates

The HACCPAC 'Reports on Demand' module includes the ability to report on Care Plan review dates and Care Plan update dates for ASM reporting by client or by system wide Care Plans.

Care Plan Tasks

The HACCPAC Care Plan allows tasks to be set as 'rostered' or 'not rostered'.  If a task is set to 'not rostered', it is excluded from the HACCPAC Weekly timesheet. 

'Rostered' and 'not rostered' tasks are distinguished by colour of the text entry in the calendar - green represents 'rostered', while red represents 'not-rostered'.

Tasks can be scheduled as 'Weekly', 'Fortnightly', 'Monthly' or 'Other'.  If 'Other', an input field is included in order to add a description.

Tasks can be viewed by 'Master' or 'Weekly' using a radio button toggle.

Care Plan Case Managers

The HACCPAC Care Plan includes a dropdown to select a Case Manager (from the HACCPAC Workers module).

Case Manager appointments and meetings can be added as tasks to the 'Tasks' table, and can be set as 'rostered' or 'not rostered'.  These appointments and meetings are then displayed in the Care Plan Calendar. Case Manager appointments are distinguished by the colour of the text entry in the Calendar (blue).

Using the Calendar 'Print' option, printed appointments lists can be created for each Case Manager for a particular day or week.  Alternatively, the Case Manager's appointments can be synchronised with Outlook.

Service Waiting List

The HACCPAC Care Plan includes the ability to add a client to a service waiting list.

The Service Waiting List includes client details, service details, date added to list and priority.

The HACCPAC 'Reports on Demand' module includes a report containing waiting lists information by client, service or household.

Client Movements

The HACCPAC Care Plan is able to record client movements, e.g. client move to hospital, nursing home, with family, and the date of the movement.  Information entered updates the suspensions information in HACCPAC (and vice versa).

Expected and Actual return to home dates are recorded in the 'Client Movements' section.

The HACCPAC 'Reports on Demand' module includes the ability to report on expected return to home date by client. 

Funded Hours Versus Actual Hours

The HACCPAC 'Reports on Demand' module includes a report containing funded hours (based on what is entered in the Care Plan 'Tasks' table) versus actual hours (based on jobs completed using manual timesheets or electronic rostering tools such as HACCPAC Mobile, Daelibs and eziTracker).
